PLEASE NOTE: each and every person who is signing up to attend class must READ this page and fill out your own name, complete address and other information as requested. This will insure proper scheduling of your first jump course appointment. If several people wish to sign up, you must complete and submit separate registration forms. You may also print the form and mail it to us.
Registration Details: A minimum deposit of $25 per person is required to reserve classroom space. If you register online we will hold your space for 5 business days until we receive payment. A $25 check or a $25 money order will only be accepted. DO NOT PAY OTHERS' DEPOSITS. NOTE: All deposits are non-refundable. You are reserving classroom space, instructors and staff to be available to ensure you on your jump in a timely and safe fashion. Erie Skydivers will send all persons signed up a confirmation for the first jump course after we receive the completed registration form and required deposit. Again, please read registration information carefully and if you are mailing us the form, please print clearly. At the start of your class the balance of your jump is due. Please be prepared at this time to pay the balance. If you have a handicap or infirmity of any type, please call before signing up for your safety and any restrictions that may involve your safety to participate. Insurance option through USPA may be purchased at Class for $60.00. Assumption of risk agreement will be filled out during class. Tandem students must be 18 years of age. Static Line students Between the ages of 16 and 18 must bring with them a notarized parental consent form. 1. Tandem Jumps: Weight limits: 220lbs (dressed weight) Height Limits: 6’2” +/- 2.
